Medicare Part D in 2026: What to Know
The $2,100 out-of-pocket cap
In 2026, once your out-of-pocket spending on covered Part D drugs reaches $2,100, you pay nothing more for covered drugs for the rest of the year. This applies to stand-alone Part D plans and to Medicare Advantage Plans with drug coverage. Premiums do not count toward the cap.
The three drug payment stages
- Deductible: up to $615 in 2026 (some plans have none).
- Initial coverage: you generally pay 25% coinsurance until you reach $2,100.
- Catastrophic coverage: $0 for covered drugs.
The old coverage gap ("donut hole") ended in 2025.
Medicare Prescription Payment Plan
This optional program lets you spread your out-of-pocket drug costs into monthly payments across the year instead of paying all at once at the pharmacy. It does not lower your total costs. Ask your plan how to sign up.
