IRMAA 2026: Medicare Income-Related Premiums

If your income is above certain limits, you pay an Income-Related Monthly Adjustment Amount (IRMAA) on top of your Part B premium and your Part D plan premium. For 2026, Social Security generally uses the income on your 2024 tax return.

How IRMAA is collected

Social Security sends a letter if you owe IRMAA. Part D IRMAA is paid to Medicare (usually deducted from your Social Security benefit), not to your drug plan.

Appealing IRMAA after a life-changing event

If your income has dropped because of retirement, reduced work hours, marriage, divorce, death of a spouse or loss of income-producing property, you can ask Social Security to use more recent income with form SSA-44.
